Medium uses the DOL LS400 “over 20 and up to 30” band (statute text says Medium for “more than 20 but fewer than 30” and High for “over 30” — exactly 30 follows LS400 → Medium). § 146-1.8 purchase reimbursement is out of scope. Tip credit and meal/lodging credits do not reduce this flat.
